Declaration of Interest Form
When we offer a visit to students via Schoolcomms, we often ask for a Declaration of interest form to be completed so we can gauge the interest for that particular visit. In order for a trip to be viable, we need a minimum number of students to attend. You should only complete this form if you have been emailed offering your daughter a place - often visits are specific to a particular year group or group of students. Please complete this declaration of interest form only if your daughter is serious about attending this trip and would take up the place if it were offered.
After the declaration of interest form has been completed and your daughter has been offered a place on the trip, you will be asked to complete a medical form. All questions are required for insurance purposes, so please fully complete the form. If you haven't the system will not allow the form to be submitted.

New rules will apply for travel to Europe if the UK leaves the EU without a deal.
You should have at least 6 months left on an adult or child passport to travel to most countries in Europe (not including Ireland).
If you renewed your current passport before the previous one expired, extra months may have been added to its expiry date. Any extra months on your passport over 10 years may not count towards the 6 months needed.
